Bridgemoor Transitional Care provides personalized, cost-effective care after a hospital stay serving as a bridge between the hospital and home. With locations in Round Rock, Fort Worth, San Antonio and Webster, this model of care from a physician-led team is designed to return patients home faster, reduce their likelihood of returning to the hospital, and ultimately, reduce the total cost of care.

The Admissions Director is responsible for building relationships with patients and their families, physicians, and surgeons, while establishing and maintaining the Bridgemoor Transitional Care (BTC) brand in the community.  This is done through identification, development and maintenance of local relationships. This role is a key partner with property team, interfacing with care team, and transitions staff, and all referring agencies (hospitals, clinics, physicians). This shall be achieved through communication of the needs to the key partners, follow up on all requests and maintain a customer service approach to securing patients for the center.

Here's what we are looking for:

  • Bachelor’s degree in Business, Marketing or related field preferred.
  • At least 2 years’ experience working in admissions/marketing in a healthcare environment
  • Experience establishing strong community relationships
  • Excellent communication and relationship building skills with a hospitality mindset
  • Proficient with computers and documentation
  • Able to use automated processes
  • Successful completion of accredited Food Handler’s Training Course within sixty (60 days) of employment

PHYSICAL REQUIREMENTS: Have the ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, reaching, and lifting up to 50 pounds with or without reasonable accommodations.
