Bridgemoor Transitional Care provides personalized, cost-effective care after a hospital stay serving as a bridge between the hospital and home. With locations in Round Rock, Fort Worth, San Antonio and Webster, this model of care from a physician-led team is designed to return patients home faster, reduce their likelihood of returning to the hospital, and ultimately, reduce the total cost of care.

The MDS Coordinator is responsible for coordinating and conducting the assessment (MDS) for all patients for a Bridgemoor Transitional Care (BTC). The MDS Coordinator will assist the Chief Nursing Officer and Executive Director with ensuring that documentation in the BTC meets Federal, State and Certification guidelines. The MDS Coordinator will coordinate the RAI process assuring the timeliness and completeness of the MDS, CAA and Interdisciplinary Care Plan.

Here's what we are looking for:

  • Bachelor’s degree in Nursing, preferred
  • Current state licensure is preferred
  • RUG Certification Preferred
  • Certified Assessment Coordinator preferred
  • Excellent knowledge of RAI Process, the Federal Medicare PPS process and Medicaid reimbursement required
  • Thorough understanding of the Quality Indicator process
  • Knowledge of the OBRA regulations and Minimum Data Set
  • Knowledge of the care plan process
  • Proficient with computers and documentation and ability to use automated processes
  • Experience working with an electronic medical record (EMR) software a plus

WORK CONDITIONS: Ability to adhere to a flexible schedule which may include evenings, weekends, and holidays.
